Medical Director
Dr. Shaya Mokfi
Dr. Shaya Mokfi received his Undergraduate Degree from the University of California and went on to receive his Medical Degree from Rosalind Franklin University of Medicine and Science. Dr. Mokfi specializes in internal medicine and has been practicing for more than 20 years. He regularly performs physical exams and is able to treat a wide array of illnesses in adult men and women. His specialties include geriatrics, cardiology, arthritis, and gastroenterology.
Nurse Practitioner
Dr. Elizabeth Hale
Dr. Elizabeth Hale has been working as a Nurse Practitioner for more than 15 years. She received her doctorate in Nursing Practice from Valparaiso University and is Board Certified in both Family Practice and Palliative Care. She is brought personal fulfillment through her career by working with and supporting her wide array of geriatric patients. She has three children and seven grandchildren and enjoys riding motorcycles and quilting in her free time.
